At this time, we are in-network with BlueCross BlueShield and Aetna. If you are covered by BCBS or Aetna and would like us to check your coverage for outpatient nutrition counseling, please fill out the form below. Anna will check your benefits and contact you to verify coverage and set up your appointment.
Insurance - Out of Network
If we are not currently in-netork with your insurance company, they may allow you to see out of network nutrition service providers with partial or full reimbursement. During each session you will be provided a "Superbill" that you can submit directly to your insurance company for reimbursement. If you would like to find out if your insurance company will cover some or all of your visit(s), please click on the "insurance script" below that we have provided to make the process easier. We ask that you fill out this form and bring it to your first session. It is your responsibility to determine if your insurance plan covers you for nutrition therapy. If your insurance does not cover nutrition therapy, our discount and specialty packages are available to give you the best results at a lower cost. Payment plans can be offered for these packages if that better suits your needs.
You may use your Health Savings Account (HSA) or Flexible Spending Account (FSA) card to pay your copay or to pay for your sessions.
Payment and Cancellation Policies
If you are using insurance and your plan requires you to pay a copay or coinsurance, payment is due at the time of service. We accept cash, check, or debit/credit card payment. If you are not using insurance to pay for your sessions, it is your responsibility to pay in full for services provided by Vivid Life Nutrition, LLC. The submission of a "Superbill" does not guarantee reimbursement from your insurance provider. You are required to reimburse Vivid Life Nutrition, LLC for any fees incurred for insufficient funds or bounced checks. If your account is not paid in full as agreed upon by both parties, you will be referred to a collection agency. You will be informed prior to your account being sent to collections and will have an opportunity to resolve your balance.
You are required to provide a 24 hour notice for any cancelled appointment. If you fail to provide adequate notice, you will be billed a no-show fee of $50. If you are more than 15 minutes late for your appointment, you will be charged the no-show fee and given an opportunity to reschedule your appointment. It is your responsibility to honor your appointment time.
